GMC Registration and the PLAB Route for International Doctors

How international medical graduates get GMC registration in the UK — PLAB 1, PLAB 2, alternative routes, and timelines.

Last reviewed: June 2026

Most international medical graduates (IMGs) outside the EEA register with the GMC via the Professional and Linguistic Assessments Board (PLAB) examination.

Eligibility

  • Recognised primary medical qualification (5+ years).
  • 12 months of post-graduation clinical experience (internship).
  • English at GMC standard (IELTS 7.5 / OET B in each).

PLAB 1 and 2

PLAB 1 is a 180-question multiple-choice paper held in several international and UK centres. PLAB 2 is an OSCE-style exam held only in Manchester (16 stations, 8 minutes each).

Alternative routes

  • Royal College postgraduate qualifications (MRCP, MRCS, MRCPCH, etc.) — exempt from PLAB.
  • GMC-approved sponsoring institutions for senior doctors.
  • EEA medical qualifications — direct registration.

Frequently asked questions

Can I get a CoS before passing PLAB 2?
Some trusts will issue a CoS once you have PLAB 1 and a confirmed PLAB 2 date — others wait until full GMC registration. Trust-grade roles are typically more flexible than NHS training posts.
How long does the whole process take?
From booking PLAB 1 to starting work in the UK typically takes 9-18 months, depending on test slot availability and the trust's onboarding speed.
